2016 Transformative Change Award – Rural Hastings Health Link
Congratulations to all 2016 Transformative Change Awards recipients!
The Rural Hastings Health Link (RHHL) is a primary care-led early adopter Health Link within the South East LHIN. The RHHL is comprised of Gateway Community Health Centre, Lead Organization; Central Hastings Family Health Team; Bancroft Family Health Team; North Hastings Community Family Health Team and broader sector partners. RHHL applied the Triple Aim Framework in developing a patient advocacy model focused on seamless transitions in care, integration, and incorporating social complexities into the patient’s Coordinated Care Plan (CCP). Their approach included population needs-based planning, performance management, developing a return on investment model, and integrating the HQO patient engagement framework to evaluate patients’ experiences. The RHHL focuses on addressing complex problems with boards, providers and patients/families, and shifting to a systematic approach that focuses on the relationships between organizations and shared objectives.
